PHP or Flash Script Needed to Generate Certificates of Completion

Hello, I need a PHP or Flash script created to generate PDF & Image files for a certificate of authenticity to provide to my customers. Basically here is what I need:
- Simple front end, no styling required.
- Name field
- Code# field
- Date field
- Dropdown or Radio button to choose the file generation options which will be (PDF or Image or ZIP)
- Generate button
What I will be doing is entering in the information of my customer, code#, date then choosing how I would like the file to be generated and then upon clicking the generate button, I will be prompted to download that type of file specified in the dropdown or radio button. If the zip file is selected, the script will need to put the pdf and image file in the zip file. The type of image that I need the script to generate will be only .jpg.
I will supply 2 certificate images. The one image will be blank, for you to use in the script and the 2nd image will be a template to show you where I want the text to be output on the image/pdf files.
If you are interested in this project please bid $150.
I need this script completed within 2 days of choosing the winner. Also, do not bid if you don't have feedback. The first bidder to bid $150 and provide a screenshot of the front-end page will be picked as the winner. This will show me who is interested in working quickly and who isn't.
