Wordpress Theme Options Page Needed

I have a custom created wordpress theme and I need to have a theme options page created for it to make changing certain elements very easy. The elements that the options page would need to be able to edit are the following...

*Option to upload the header image to use

*Text field to change name of company (appears towards top right of site)

*Text field to change the list of "Areas Served" (appears above company name)

*Text field to change message and phone number below company name

*Option to make opt in box optional (this is the "free tips" section on the right hand side)

*Option to paste in code for aweber (this is the code for the "free tips" opt in box)

*Text fields to change names (anchor text), not links, of the footer links.

*Option to upload the "Who am I" pic w/ instructions on the size it should be (this will be provided)

*Text box with character count and a limit (limit will be provided) for the "Who am I" description

The deliverables would be the new wordpress theme files with the options page included.

I can either set up the template you'll be working on on my server or I can send you the files.
