Hello, i need a programmer who can connect my existing webpage (php/mysql)

with asterisk.

Process :

Call comes into the asterisk, asterisk does the lookup of my mysql database if customer exists, then fills up the fields on my existing form for that customer.

Also i would like to integrate softphone if posible on same webpage.

Its like a small callcenter app.
