I need some software created for a shipping website.

The program should allow a user to enter in their Zip Code and the Zip code they wish to ship a package and receive a quote for the shipping cost.

I would like to also allow them to choose the size package and if its residential ro business address.

Should be an easy form to create. I am able to create an excel spreadsheet that does the same but not sure how to make it work on my wordpress site.
