I am looking to create a website contact form that is mobile and spam compliant, however it also must appear to be coming from an email address under our control so that emails do not end up in the spam filter.

It can be written in PHP or similar etc.

DIFFERENCE: normally contact forms (come) from the email address of the sender, however I am looking to script the form so that it (looks like) the email is coming from one email address under our control. This is so that various emails being sent from the website contact form do not get caught up in our SPAM filter / folder.

Since the web form email is now coming from (one specific email address) rather than from the customer's email address, we can then modify the Outlook junk mail filter to prevent incoming emails from ending up in the junk folder.

I hope this all makes sense.

The code will need to come with an explanatory data sheet if not self-explanatory. I want to be able to use this contact form for many websites. It must use the latest code and be fully compliant. I also want it to have CAPTCHA.

FIELDS: In the contact form I will need the following fields:

Name *

E-mail Address *

Phone Number*

Company*

Position*

Location: (Region / Country etc)*

Subject Selector* (this will be a drop down selector)

Message*

Captcha
