Website and Server PHP Optimization - PHP/SQL Hangs Forcing Restart of Services

Hey there!
I'm in need of someone to help me optimize my website and server, specifically relating to a php/sql issue that we keep having, mainly relating to php-fpm pools not clearing out and forcing us to restart php and mysql on one domain. The attached image shows all of the pools staying open with little load on the 12 core server. It causes the website to hang and not respond. If we restart the php and sql services all is good again. It usually happens once or twice a day, not always directly related to something we're doing or traffic. The site is admittedly large (almost 4,000 posts and 5,900 items in the media library). I have root access to the server and can provide access; however, I want to know about all of the changes that will be made before they are made.
Ideas that I think need considered (but I know just enough to be dangerous):
How the pools are created and maintained (dynamic vs set) - See * below
