I need you to develop a data entry software

I currently use a swimlane process to input all my information and would like to further develop into a usable software which can be used online

1. Same as swimlane process used in excel

2. Data input into lanes seperate

3. Once data has been input the information is automatically uploaded into the correct lanes

4. Each lane is labelled

5. A report is generated along with process
