Custom Outlook Form / MS SQL Server

I need a custom MS Outlook (2003 compatible) form based on the Contacts form that will connect to a SQL Server database contact table with columns as in the original form.

This is a quick and easy job and I'll pay $15.

You provide me with:

1. The form that I can import into Outlook.

2. VB Script code that I can use and edit.

3. Connection string that I can update to use on my SQL Server.

4. SQL Script for CREATE TABLE contacts.

Needed by Monday 5:00 PM US Central Time.

If you can't do this, please do not bid.
