Opt-in page [login to view URL]

Hi

I am looking for someone with experience to set up a very simple opt-in page as the attached.

Wording and design are shown in the attached and I will be providing free royalty pictures to include.

The domain is already bought and the website should be able to:

1.-Allow user to input email

2.-Capture email and keep it on an accessible database where all the gathered emails would be added up.

3.- Send back an email to user (this designed as per attached) and send a .pdf document to the inputted email.

4.- Set up Wordpress so any texts or images can be easily modified at some point in the future.

Note that I have an account with Amazon S3 so I understand the database can be easily linked to an Amazon 'bucket'.

This should be discussed.

I expect the person to have experience on this short of set ups and guide me through all the above steps.
