The study was conducted in the Kalutara district situated in the Western province of Sri Lanka. This district covers an area of 1028 square km. The estimated mid year population in 1998 was 1,031480. Of the population 12% live in urban areas. This district is divided into nine Medical Officers of Health (MOH) areas, which also includes the field practice area of the National Institute of Health Sciences. The Kalutara district comprises 296 Public Health Midwife (PHM) areas and 763 Grama Niladhari (GN) divisions. The GN division is the smallest population unit identified for local government administration purposes.
