Database Creation and Entry of Data

For many years, healthcare practitioners have been registering to use my password protected site. I have simply filed the email notifications in either a hard copy or digital format which is not searchable by specialization or location, the chief question others ask me when they want a referral. I would like someone to create an aesthetic, easy to navigate database that is searchable by fields. I am attaching a copy of the form used by applicants. The only practical way for me to provide the input data is by batch forwarding a lot of emails or copying and pasting them into something like MS Word. My experience on RAC is such that it would not surprise me if someone else has a better idea. I do not honestly know how many applications there have been. I am going to guess 300-500 so I will ask you to bid on the basis of 300 and have a quote for incremental batches of 50. Attached is a copy of the application. Since no other questions were asked, these are the fields required. The second part of this project is to harvest the emails and send notifications to all previous applicants, alerting them to changes. The third is to develop the database in such a way that it can be exported into an admin panel online, yet to be developed. Thank you very much for considering this project.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
