Building a questionnaire in Google drive

I need a freelancer to assist me build a questionnaire in Google drive. So, i could share the questionnaire link with the key informants. Key informants are allowing to response the questionnaire items, BUT not to change anything in the questionnaire itself. Notes: 1- I have the questionnaire in excel sheet. 2- I set the response, for open and closed responses. 3- I have a gmail account. 4- the questionnaire in English language 5- fixed price, 5$ 6- I will give you all what you need to do the job.
