The electronic sheet is supposed to be a short test Yes or no style. All test takers should be able to take the test by inputing a link. The test taker will input his personal requested details and will take the exam. The exam result will be conveyed to a dashboard to me where I can see all the answers. The tests will also be marked such that if a test taker scores 8 or more questions as "YES" I should be notified of him/her either by color coding (8 or more Q as YES=RED color) or in a separate sheet showing all those that scored the pre-defined limit. The test should allow for ease of access and the information gathered should also allow for ease of analysis. A simple flow diagram is attached.
