Design me a slick portfolio (PDF)

I am a UX designer and I need someone to make me a slick looking portfolio for m * * * y work. This will be a PDF document to be read either on-screen or in a print-out. I will provide the high-level structure of the document, all the copy, and all the work samples (wireframes, process flows, personas etc). The rough structure will be: > Cover page & vitals > Introduction > Projects 1 info (some intro copy & captioned work samples) > Project 2, 3, 4, 5 (as for project 1) > Other work samples > Work history & skills You will put it all together in a way that looks good. As well as the document itself, there will be one or two deliverables that I will need you to convert from their current 'raw' format, into a bit of a slicker looking one (e.g. a persona) I want a very quality product so I am looking for someone with experience in this type of thing, and I will welcome ideas and guidance about presentation and structure. I will pay accordingly - I hope that the whole thing including the initial briefing, the work itself, feedback and revisions should not cost me more than $500, but I am doing this as a 'pay for time' project because I would rather pay a bit more to get this right, rather than have the designer fuss about making revisions or spend time discussing things.
