Designing, formatting and automating document is what I do working for an insurance company. Insurance companies like to collect a lot of information to evaluate risks and it is paramount that forms are easy to fill, easy to save, easy to send back and finally, easy to extract into Excel or a database. In addition, I have completed multiple forms for a medical doctor through Freelancer. Three of those forms were over 16 pages long with complex information gathering. My proposal will meet all the requirements you have listed in the project description with unlimited edits until you are fully satisfied with the finished automated form.
