Hi There, I have a 3 page PDF gym membership form (attached: "Par Q Form"). Currently we print this out and fill out in pen. I would like someone to create a version with editable fields/checkboxes/radio buttons which can be filled out on a computer (and then can be printed out, or saved) In addition to this, I would like added to the top right of the front page some checkboxes (see below): Office use only: [ ] Member Records [ ] Digital Copy [ ] EZYPAY/Payroll [ ] Member copy [ ] Gmail [ ] Keytag#_____ [ ] Mailchimp [ ] Intro Email The area in green on the front page may have to be reduced in size to accommodate the checkboxes. In case, you're not sure what a editable PDF document should look like (not mentioning names...) I have attached an example (attached: "EZYPAY direct debit form") If you cannot create a document of similar quality to this, please do not bid! Thank you!!
