1) Using the existing form, develop 2 address fields: "from" and "to" and draw directions in a map using google maps api [login to view URL] (We are currently using the plugin VFBPRO to manage forms) 2) The 2 address fields must have google autocomplete feature 3) The two addresses and map (as a picture) must be included in the forms submission data 4) Provide customized form submission confirmation. The ZIP code of the "from" field must be checked against a table hosted in Google Sheets. This table has two columns: ZIP and KAM. The KAM is just the name of the account manager responsible for that ZIP region. The system must find the correspondent KAM according to the ZIP information. Upon form submission the confirmation must show the KAM name. 5) Upon form submission the wordpress user with exact name as KAM must receive the form submission as a PM (private message in buddypress). All KAM names are previously registered in wordpress.
