A Phonegap App for Travel Site

I need to get a train ticket booking application developed using PhoneGap. The essential features are the obvious travel domain features: Selection of stations, dates, travel classes, and showing of availability followed by actual booking and payment. Customer will also have the ability to save "FavoriteTrains", "Favorite Travel Classes" and so on. The application will need to download some data at the beginning (activation) and keep it for quick access. The back end communication is over http (login is over https). Most of the application will consist of user submitting forms with quickly accessible combo boxes, list boxes etc. Some forms will be complex (like booking) and some will be simple (like add a Passenger). There will also be simple screens which show information (like list of tickets), but even those screens will have some working functionality (like check status of ticket) . A couple of static screens (like about us, terms and conditions etc). Not counting alerts. Work will need to be submitted on a daily basis. I am looking at full application on the client end, UI elements need to be updated according to data received from the server. The back end is under our control, so whichever format you wish to have as input, it can send that. My expectation is that you will at least test it on Android. Code needs to be readable and customizable by local developers later. I am looking at pricing per screen. Either one flat pricing per screen, or if you wish, you can provide different pricing for simple forms, complex forms and static screens. I am looking at a tight timeline. So please include in your bid how long it will take to create, say 5 complex form, 10 simple forms and 5 static screens. [Attachment: Attached is an example of a simple form.]
