Contact Form

I need an HTML contact form built based on the attached word doc. The fields in yellow should be check boxes. Everything else that's a checkbox should be changed to a dropdown list. You will need to change the layout into more of a list and make sure it fits into the width of the attached ASP page. Put all of the text into table cells (TD) as we have a style on that. The fields in the word doc need to be added to the HTML form that is on the ASP page. Add a file upload option to the end of the form to allow the user to attach a file to be sent as well. Use ASPUpload as the upload component. Then code the page to send an email when Submit is clicked. If there is a file uploaded, it should be attached to the email. After the mail is sent, show a message on the page in red that says "Thank you. Your message has been sent." I need this ASAP.
