(1) Web server Program AZ for online item evaluation based on original code using php with mysql. (2)A user registers, logs-in & begins to enter his/her answers to 250 specific statements, one at a time.(3)There are 13 categories that comprise the 5 stages of AZ lifecycle.(4)Each category has been assigned a relative weight, ex. category A1=50 pts. Each of its 14 statements equals (50/14)=3.5 pts. (5)The user is asked first to enter his/her score (scale:100 = Best;0 = Worst) and then justify his/her score by entering 50 words, max. (6)Only one statement at a time must be displayed on the screen for a limited time and its displayed sequence from user to user must be random.(7)Administrator must be able to add, revise or delete statements, categories or stages or the relative weight of each category without affecting the program’s calculations and/or performance.(8)Each of the five (5) stages of AZ lifecycle has been assigned a total, BEST score. For example, BT1 is assigned 437.5 pts.(9)Once a user has entered his/her scores and respective justifications for each of his/her answers, then the program must compute a total score.(10)The program must compare the total score per item 9 against the BEST score that is specified in each of the 5 stages of AZ lifecycle and display the result for each category and overall total. (11)If more than one users have entered data for the same AZ item, then the program must red-flag any category that its score varies by more than 5-10% from user to user.(12)The program must offer the user the option to receive a detailed report for a fee. Then upon online payment to a secure website, the program must register that request and forward that data to a specified email address for subsequent evaluation. ## Deliverables 1) Your bid MUST include intelligent and probing questions that can demonstrate to me that you know what you are doing both intimately and thoroughly, thereby you are fully capable of completing successfully this type of project OR your bid will NOT be considered. 2) Each selected bidder will be required to sign a Confidentiality Disclosure Agreement about the subject Intellectual Property of Costas Chantzis. 3.
