Ajax form

I need an Ajax form that ask questions 1 by 1, with this information: Province Name E-mail Phone (only 9 numbers) Category (from a list editable) Type (from a list editable) Title Text Prrice Images: Yes/No Image 1 if images yes Image 2 if image 1 or skip Image 3 if image 2 or skip Image 4 if image 3 or skip All correct Then save information at MySQL database and send email to email adress with a link to confirm. I need all the questions asked 1 by 1 with ajax. First question (name is asked) and there is a help for this question. When first question is answered, then ask the second question, with the second question help showed. First question help is hided, but first question is showed up above with a "correct" button to change the question if necessary. Then all the questions the same, with the possibility to change all the questions above, without lose all the data filled, and continue at the last question when the question changed. All with Ajax (same window, like Gmail, you know) Must be in php with MySQl database. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
