Data Correction Form needed

I operate a database that tracks client informaiton (such as gender, race, income etc...) Users are locked out of "editing" any record so I've designed a form "frmMAINITDepartmentcorrection" that I want users to use to ask me for a correction to be made. I want the correction form to ask the user what screen they were on, then what seciton on that screen, then what question etc... (you'll understand when you see the form). Based on a users responses to these questions, I want the answer they want changed displayed and have them choose a new answer (seperate question). All this info is stored in a table "Client Corrections" for me to review and change myself. NO info is to be changed. just added to this table. I've included what responses I want available based on a users response.
