Custom build bulk database upload script in PHP / MySQL

Hi, I have a online yellow pages website. I want PHP / MYSQL Expert to create a small standalone script of bulk uploading the business listenings and dump into database directly. Script should have admin userid and password to access The script should able to allow the following 1. Select file CSV from computer 2. Dropdown field Load automatic Categories from category table and add category id while uploading the listening. 3. Dropdown Sub Categories under main category and add sub category ID while uploading the listening. 4. Dropdown Country then state then city the fields from my excel sheet should map along with above mentioned Category, Sub Category, Country, State and City ID selected from dropdown and upload the listenings into business_master table. There is table in mysql named "business_master" Business Master Table contains following fields id (auto increment) mem_id title (from csv file) category (ID auto get from dropdown) sub_category (ID auto get from dropdown) country (ID auto get from dropdown) state (ID auto get from dropdown) city (ID auto get from dropdown) locality pin code (from csv file) address_1 (from csv file) address_2 (from csv file) fax (from csv file) toll_free (from csv file) email (from csv file) website (from csv file) phone (from csv file) phone_2 (from csv file) contact_name_1 (from csv file) contact_phone_1 (from csv file) contact_name_2 (from csv file) contact_phone_2 (from csv file) status verified viewed created_on (from csv file) created_ip agree description (from csv file) services (from csv file) logo type priority long (from csv file) lat (from csv file) like I have attached all the tables in SQL for you to understand the structure. The script should show the preview before dump the final data into mysql tables. I cannot give access to the whole script or database directly due to security reason. Thats why i have attached the SQL tables how they look like.
