Convert flat file into XML with different layout

We receive a monthly flat file from one vendor that we need to upload into a 3rd party application in XML format. The layouts are different from the flat file to the xml file. A sample flat file and specs on both layouts can be downloaded from: [[login to view URL]][1] You will notice that Field4 from the source file is the same field as Field2 in the CART xml file. It also needs to be changed from mmddyyyy to mm-dd-yyyy format. Also, the source file has the Patient Name field in 1 field in SMITH JR, JOHN,J format. However, it needs to be converted into both a First & Last name field in the CART xml file. The first field in the CART xml file is "ImportType" and is not located anywhere in the souce file. We will need to always have this field set for either CMS, JCAHO or ALL. I'm not yet sure, I'm still trying to find out. There are plenty of fields from the source file that will go unused in the CART xml file. This is ok. We will not need them. This does not need to be any type of full blown windows gui app. A simple command line / batch interface is fine.
