convert an existing excel table into platform independent apps or Crossplatform , can run in any iphone/ipad/android or windows, lunix apps with addess features

the file attached is simple excel file that need conversion into a cross-platform [login to view URL] install and run in iphone/ipad/android tablets/Mac OS windows OS. how it works: individual map number is issued to people, the map date out is recorded, what campaign (if any) the map has been worked on (max 3 char in C column). Then once map is returned the date-in is also recorded. A form similar to the attached one is to be printed out once full as its current format A4. Featured: need report on how long/how many maps currently out, report on what campaign and how many maps has being used per campaign. report on what maps can be used next upon criteria of 4 weeks(editable) after returned date. report on overdue map with editable criteria of 20 day out. NOTE: ability to add and report due date or number off date out =20days (note due date field need to be editable) report on overdue maps. ability to add and modify maps numbers without loosing history. ability to to email overdue maps to precipitants or option to SMS overdue maps. if any more question please let me know.
