I need a simple web form that allow employees to submit a weekly timesheet and upload an XLS file with the submission. The XLS file will have job numbers that the employee worked on each day (one number per field). The job numbers will need to be parsed into the database and checked for duplicates of already submitted forms (from any submitter). If a duplicate is found, the submission needs to be rejected and the submitter notified of the duplicate field(s) value already submitted. An XLS report with the data submitted will need to be developed as well.

Hello, I'm assuming the "job numbers" are entered across the rows numbered 1-100. I also assume that the database is already existing. The process of submission and checking the XLS file for duplicates are fully understood. Is there an example/template of what the XLS report would look like? (1) What fields in the spreadsheet are to be used for populating the database? (2) Is the spreadsheet standard, and unlikely to change in template? (3) Does the spreadsheet have only one sheet.
