Create a landing page for health quiz

We have had a 2 part quiz developed that is intended to capture leads and also give us key statistical data. We supply vibration exercise equipment, and the quiz works as follows... Part 1 - A crude biological age test, calculated by answers provided on a number of general health questions. Part 2 - A series of questions designed to determine whether the user is suitable for vibration exercise, and if so, which model is best suited. We ask the user for their name and postcode before they can proceed with part 1 of the quiz, and then we ask them for their phone number and email before they can proceed with part 2 of the quiz. We will direct PPC traffic to this quiz via a squeeze page, and in addition a link to the quiz from website. The quiz has been put together by another creative who has limited time at the moment, and it is not quite complete. But you get the idea of the quiz here: [login to view URL] For the purposes of this job, I would like someone to design a PPC squeeze page intending to entice people into doing the quiz. Since the quiz itself captures the user data, we don't actually need a capture form on the squeeze page, we are just trying to get them to click through. I am looking for someone who can design an attractive page, and also create compelling copy. Right now we are hunting for a creative who will get to know our brand and product, to work with us on an long term basis. We have released a number of new products, and are expanding into new markets. In the coming weeks we want to overhaul our brand identity, website, printed collateral, and much more.
