Design a database to capture information

Please correct my understanding. The requirements is to create a web application where users(staff) can can enter an incident and after that take report of all those incidents. But from where tha data of these incidents will come. Do they have to enter manually? Each incident will have field as date, time, location, officer name, body etc. After this they have an option to send secuirity report to specific people. I have understood it partially. Need more discussion. Thanks

As I understood, you need a database that stores your business related data and GUI (web or windows based) that allows users to enter, modify, search information as well as created reports.
