Create a website to generate PDF ID cards

I need a website form to generate 3.5" x 2" PDF files for identification cards. All user records will be stored in a database. To create a record, the user should input: first name, last name, position, and upload a photograph. All these details should be stored in a database. The photograph needs to automatically re-size to 1" x 1.5", regardless of the size of the uploaded photograph. The page also needs a search function, to search for an existing user. When an existing user is found, a button marked "Generate PDF" should generate a 3.5" x 2" PDF for use in printing. I will provide a background to use for the PDF, but the photograph, first name, last name, and title should appear on the PDF. I would prefer this project be done in PHP and PostgreSQL, but am open to other languages.
