Oracle Application Express (APEX) application required

A small Oracle Application Express (APEX) application is required. We have a bar code reader that will be used to scan a barcode. When the barcode is scanned, we would like it to record the scan into the database table EMP_SCANS We have one table in our application and below is the script for it: CREATE SEQUENCE "EMP_SCANS_SEQ" MINVALUE 1 MAXVALUE 9999999999999999999999999999 INCREMENT BY 1 START WITH 1 NOCACHE NOORDER NOCYCLE NOPARTITION / CREATE TABLE "EMP_SCANS" ( "ID" NUMBER, "STATUS" NUMBER, "SCAN_NO" NUMBER, "DATE_CREATED" DATE, "CREATED_BY" VARCHAR2(100) ) / CREATE OR REPLACE EDITIONABLE TRIGGER "BI_EMP_SCANS" before insert ON emp_scans for each row begin if :[login to view URL] is null then select [login to view URL], sysdate, user into :[login to view URL], :new.date_created, :new.created_by from [login to view URL]; end if; end; / ALTER TRIGGER "BI_EMP_SCANS" ENABLE / The Oracle Application Express form that appears should only have one form with one field on it: SCAN_NO When the barcode is scanned, it should get automatically inserted into the table EMP_SCANS (there will be no submit button on the form), and the form should go to the new record input mode, thereby allowing another scan to be recorded. Attached is an image of the form we require. As you can see this is a very small and simple application.
