Build a custom wordpress multi-step lead generation form -- 2

We need a custom wordpress multi step form for lead generation built asap. We have the format wireframed, all the creative and copy zipped up and ready to go. We like to make it as easy as possible. Here are some of the features: -Open sans bold + regular fonts. -one color text -png background -12 -14 steps -14-16 questions -Image select questions (no button - user selects / clicks image > next step) -slider bar questions Drop down questions Text questions Fully responsive 1 or 2 conditionality questions Prefer steps to be on one page /url only but not nessesary. etc. etc. We dont care which technology / form builder is used.
