I need a web form that people can fill out and it writes onto an already existing template file (which is attached). The person would fill out their name, job title, telephone number, cell phone number, email address, and website. These would then be added to the PDF, which would then be available for download. The form and download button should all go on one page, and the download button should appear when the PDF is created (using AJAX for no page refresh). Must use vanilla PHP without frameworks like Codeigniter or Laravel. The font for the text that gets added should be Google Font Roboto, which you can download here: [login to view URL] Can use a PHP library for creating the PDF. Must be completed within 4 hours.
