Form Part - Charity website for Homeless people to get into housing

Hi, i work as a volunteer at a homeless shelter. I want to sort out a website for homeless people so when they move into a house i come in and check it out that is all ok and make an official report. This is a sort of complicated thing but im working on this from my own money to help get the homeless off the street. I am working ion the site myself and need some help with sections of it, I have been ripped off twice now on here from developers saying they will help cheap and pay money now, im just to kind and trust people. Freelancer wont let me get my money back even though they can see the messages from them saying they are not going to do it. Anyway what i need is Website ive made with the latest wordpress. Form / propagation form This is where users sets up the form and it propagates into an admin check form. The is when user "Tennant" creates a new application form. When this happens it shows in the tenants current appliction form list. When submitted it also creates an admin check form. The idea of this is when i goto the tenants rental house to inspect i can tick boxes, make comments and upload photos for each room. If you open the application form file you will see the tenant application which is basic. Then when they submit it creates an admin one, the admin one is based on default parts(see image) this is named and labeled in the description as you can see. So if a tenant picks 3 bedrooms each part is named to that for all fields, like the image shows. Checkbox. The check box is on each partn. this is controlled by the config file. So i first setup the config file for the room type, so if i tick fan on bedroom and Toilet then it shows Fan in them 2 room "Parts" on the admin application. I am happy to discuss more to get this right if you dont understand fully.
