Lead entry system/website PHP requires modifications and improvements

Need a developer to help complete unfinished project and making modifications to existing project. PHP and Angular a must. We will be working on an existing lead and CRM management website and making changes/improvements. Adjust our lead entry form to have all the values of the attached sheet. Lead page as shown must have the ability to print out the content of a given lead record, you must provide this feature. Lead page screen shot is attached. A button for print and another for PDF would be ideal on this page. Printout should somewhat resemble attached lead sheet, but with all the information of the leads in our system. I attached the form which should be used as a basic model for the property information screen shot shown. Be sure that the form has all the fields of the attached sheet at the minimum, if there are additional existing features that is acceptable and to be left alone. Obviously there are additional and unique fields so I'm not suggesting this be used identically, just as a model representing some additions that need to be made to our existing form. Change the header and order of lead form tabs as follows: Currently the header of each tab contains the look-up fields for Name, Match code and Property address. Since the look-up function targets entering a new record, its less helpful, while editing a current or past lead, to have it on each tab. I would rather have the records identifying information at the header of each tab. These would include the current records Lead ID/Number (remember we are adding that), the Owner Name and the Property address. I suggest that when adding a new lead (as happens when you click the New Lead button) the system would present the current look up fields (as it does now) and the option to bypass the lookup function if a prospect record is not found. It would then allow for the direct entry of the basic lead info (Probably just property address and/or name) which would be saved, creating the record, and then would go directly to the editing tabs as they exist now (but with the id fields at the top vs the current lookup fields). I'd also like to change the order of the tabs. It makes more sense to me to default and start in the Property Info Tab. The Lead Info tab could be moved to the last position, behind (to right of) the Investor tab.
