I made a mistake whilst trying to edit a contact form on my wordpress site, and some of the form deleted. Please can you bring back the previous version of the contact fields in the old form (old form file, the top part of the form is cropped out in the image). Also, some of the text may need to be changed in the new patient special form on my site. Changes are: Line 1: change "NEW PATIENT SPECIAL!" to "FREE INITIAL VISIT" Line 2: change "£5 OFF YOUR INITIAL VISIT" to "INCLUDES CONSULTATION & ASSESSMENT" Line 3: keep "Offer available until 30th November 2018" I need this work done in the next hour please. I will only accept a bid of $10 as i know it is a very quick job.
