Wordpress popup with mysql data used to populate another form with double click

I need the php files used to create the Wordpress shortcode (add_shortcode) that have the following functions: It creates the shortcode for a four field form (at least one int or text) that updates a mysql table. When the user clicks on each field in the form a popup window opens that provides choices that themselves come from a mysql table from the same database. As the user double-clicks on one of the popup choices, that fills out that field of the original form. There is then a submit button to finalize the process. I need just the php file, [login to view URL] and minimal css file that i will copy into the WP_Content folder then deploy to wp page with the shortcode.
