Create a simple landing page for GDPR Compliance with a simple 4 columns table in database for data gathered

Project ETC is 3 hours Budget 30 euro For a PrestaShop platform I need a new CMS page where customer can agree to receive marketing messages or unsubscribe from the list. We must show a text message, 2 radio buttons for choosing subscribe or unsubscribe options, 4 text fields with Name, Surname, Phone and Email and a send-your-option (finalise) button, after finalise there are 2 options: -if subscribed (accepted) to receive marketing materials "Thanks" message and auto redirect after 3-5 seconds to main page -if unsubscribe then confirmation message and close page. All information from 4 text fields must be stored in a simple 4 columns table with possibility of export infos as CSV or EXCEL. Attached you can see a preview of what i need. Infor from that 4 text boxes must be stored in DB in a simple 4 column table, with the possibility of export data in CSV or EXCEL. Also, Phone Number Box will be MANDATORY and send Button enabled only after filling the Phone Text box and checked either one of the radio buttons.
