Hi, I'm an electronic engineer with more than 5 years of experince in embedded system development and Arduino programming. As per you project description and pictures attached I can see you are using Arduino Uno. What we can do is, connect an Bluetooth module to Arduino and send data via BT to an Android app. On the app we can show charts, and any other way you want to show data on. But if you need to send this data to an app from a different location, we can connect ESP8266 to arduino and upload data to a cloud server using WiFi. Then we can get the data from the cloud server to the app and sho in the app and a webpage as well.
