Patient Registration and Prescription Software

I am an Orthopaedics Surgeon. I want a simple to use Patient registration and prescription software. Requirements: 1. Can run in an Windows laptop/ desktop 2. The starting interface should be able to capture some basic patient data such as Name, Age, Sex, Address, Date etc. 3. Space to write Patient presenting complaints/ symptoms 4. Should be able to create some disease specific templates and to insert them as and when required. 6. Prescrption writing where the Medicine brand databases should be able to add to database (dont want an vast online database), the brand names should be autocomplete when typing and the doses should also autocomplete once the particular brand selected. For example, lets say I add 10 brands to create a database, including their doses. Then if i start typing anyone, say "Pantop40", it should autocomplete once i type "Pa". Secondly once i select it i.e. move by hitting "TAB" button, the already set dose should appear in the next line. The other details we can share once the project starts. Thanks Firoz
