I have already done a similar software,but it was not for school psychologists. My existing software contain 1- Disease definition 2- Client Definition 3-Doctor(Psychologists) Entry form Different values are taken E.g , Blood pressure,Cholesterol,Sleep Condition and various stuff.. 4- Predefined values are already defined for these conditions E.g blood pressure = 120/80 and based upon this value patient data is calculated and a mean Score will be presented in tabular as well as graphical form. 5-History of patient with last 5 visits/scores is also there in a pie chart/bar chart form.
