I have a PDF application form where customers can fill in their details, save it on their computer, attach it to email and send it to me.
Now I would like to get this application form on our website, and sent directly from our website to my email. I was thinking to have this form on its own clean emty page on my website with nothing else on this form, and when customer fill it out and press send, I want the information sent to my email as an PDF file. In other words, I want the information from the website copyed and past into the correct fields in the PDF file.
The application form has 21 fields.
