Brochure template design for a private healthcare clinic

We need a brochure designed for the attached program we are running. This will be used internally in our physiotherapy practice to entice clients and to give information on the benefits of the programs. We want it to really sell the program for us. The attached XL document has the text for the brochure on one page. I am imagining a fold out DL size brochure (doubled sided A4 folded). We have a recent flyer (attached), from which we want to use the photo. I would also like 2 similar happy photos that would appeal to males (eg tradesperson) and athletes. Options of photos to choose from would be best. We like to use recycled stock with a slight grain, so the images need to work on that. Looking for a modern but earthy feel, and it needs to be focused on selling the program. the table in the brochure should look similar to a table used by software companies to show benefits of upgrading. The second page on the attached XL sheet is a current price list, which we want to be able to freely change ourselves as prices change, and to print off and insert into the brochures. The design needed for this must be easily editable by our admin staff who have no design skills or software. We also want the brochure to be a great template for our other internal programs, so that we can edit the headings, text and pictures freely. Attached are: our logo (different colour versions with the massage text, but it needs to be 'physio' - open to either, though we are just changing the signage to the dark background version), a recent flyer we had designed to promote this program (plus massage on the back), letterhead and business card stationary (note we are not using the second address on there). We need unlimited changes, and applicants need to show either mockups of design or examples of similar work. Payment will be in full once the project is completed.
