First the consumer uploads their photo. Then they put placemarkers on their eyes, mouth, etc. Then the hand comes out and gives the face a "slap" Then they see their photo distorted so their face looks like it got smacked. Then you should be able to share the photo via Facebook, Twitter or email. And then there can be a screen where people can click on links to a website, etc.

Hello, I think I might be the perfect freelancer for your project because I created a similar face distortion App once before. I can probably use some face recognition so that the user does not necessarily need to place the markers by hand , or at least I can try placing them and then they can adjust them if they are not correct.
