506203 Life Insurance Calculator Script

Need a simple life insurance calculator coding in something like Javascript to be placed on a html page. The style must be similar to the Life Insurance Calculator at [login to view URL] Use javascript, html, css. 1. User chooses steps 1-4 2. User clicks compute 3. Shows the amount of life insurance recommended and displays the hidden <div>with 300x250 image and hyperlink</div> Have included images of what I'd like for it to look like. -Must be in text (not graphics AND editable) -Can edit text size and font in CSS -Amount is blank until "compute" button is clicked -Hidden <div> that shows "Recommended Insurer" and 300x250 hyperlinked image shows when "compute" button is clicked
