Create GUI and prepare reports from mySQL

Need to create a Login form so client can login to see his own vehicles various reports in 2 catagories (Sensor Based, Device Based). Reports depends upon the user rights, what he or she is authorised. (SA can control the reports for Admin, Admin can control reports for Distributor and User) Need to get reports based on sensor (Ignition,Door, AC,Alerts,Fuel, Temp,Idling,Work hour,Milk tank) Device Based (i.e. History, Movement,Stoppage,trip,Distance(All),Weekly/Daywise Distance,Overspeed,Speed,Monthly log,Summary,Catagorised Summary,SMS) You need to make sql querry and get the different reports in tabular and line/bar graph format. I will give you mysql data filled and GUI, so testing can be done at ease to final bidder. Each report should be capable of Display on screen,Excel, Pdf, Email (excel view)
