Hello,
I have several websites online that collect leads, and i need a database where to store them for
future marketing purposes. Initially i was looking into autoresponders, but they are somewhat limited
in the amount of data i can collect for the segmentation of my list.
1) First of all i need an secure online form which can collect the data and send it to my database.
I should also have the possibility to prepopulate the next form, after the first form. Prepop means that if a user submits his data
on a form, i can then take his data and add it automatically to the next form, so he can complete the registration process.

2) The database needs to be hosted on a website i own.
The database website, is where i can see all the leads, and the data about them(age, country, zip, etc.),
from all the websites i own. So, no matter how many websites i have or will create in the future, this website
is like the HQ of what is going on on my business.
I would also, like to implement this dashboard into the website, so it will have a nice look:

Let me know if it is possible to implement this dashboard with the php and mysql database.
3) I need the website to have it's own API, so i can connect to other applications and get the data
from there pulled automatically in my dashboard. For example, i want to connect to google analytics
to get data from my traffic inside that dashboard.
Or to be able to connect to cpa networks, or different businesses that API, and get the amount of
leads, sales and revenue i made for them, directly in my admin area.
If some of you have questions, please let me know. If the budget is too small, please give me a quote
for what amount of money you are willing to do this.

Point 2 is possible, but you need to let me know how complex you want your dashboard. The example you gave is way too much unless you really need all that kind of information, and we should talk again about the budget and time.
Point 3 should be fine.
Overall, this project should be fine and should be able to deliver in +/- 20 days.
