Create a page for profile registration and a login screen with password reset functionality

The requirement is to 1) Create an HTML page for login, password registration and ability to change password via a "change password" link/ button. Password change will be allowed on-line by letting the (logged-in) user supply the old and new password. A "forgot password" link will allow user to send a request to change password. In this case, the user will be asked to provide first name, last name and email id. An email will be generated for the user containing the new temporary password which the user will use to log in and change by logging in. (2) On successful login, the user will be allowed to securely access a profile page, enter/ modify several fields that express his/ her profile (e.g.., name, email, address etc). There will be check boxes, radio buttons, text fields. Some fields such as Country of residence would be a drop downs. All details of profile will be entered on a single HTML page. Sample document containing all fields on the form will be provided to you for designing. The profile page will also have an image area and a file upload area where the user will be able to upload (or change) a picture and a file (word/ pdf/ text document). These files (image and document) should be stored in a pre-designated folder under the root folder. The page will also display an embedded area that displays an agreement to terms and conditions. If the user agrees to terms and conditions and clicks submit, the page will be saved with all details on a mySQL database. You will have to create a table that stores all details of the information provided in the profile form. The user will be sent a confirmation email about successful entry (or modification) of profile. Additionally, a button on the screen will allow the user to enable/ disable the profile or delete the profile completely. On doing so, an email will be sent informing of the action taken (profile enabled/ disabled/ deleted) in the database. Noe that whenever the user logs in successfully, the profile page should display the last mage uploaded by the user and the name of the last document (if any) uploaded by the user. The HTML page including login should be compatible for use on PCs, laptops, smartphone (iPhone/ Android) and other mobile devices. These are the overall requirements. If there is anything missed, it will be explained but any other change if any, would be minor. The successful bidder should preferably be using MAMP (latest version) so that code may be tested on a Mackbook pro. The successful bidder should be open to change of requirements such as placement of field or the number of fields/ buttons, the overall look and feel of the page, order of fields, fonts, formatting etc. The successful bidder should be aware of secure access methods and be able to provide a solution that prevents hacking of the user's profile information, userid and password.
