Desktop Application

Need a small desk top app to keep track of the pizza store inspection report. This application will be installed on laptops and is to keep track of Pizza Stores biweekly inspections. During the inspection, the inspector finds violations and at the end of inspection writes a report. The store manager signs the report through an attached Digital Pad. Developer will download the API for the given model from the digital pad website and integrate API in the app. The application will remind the user(popup window) to follow up upon launching the app or upon entering the store name or number. Application should keep complete history of all the visits for each store. There are 200 stores. All the reports need to be Time Stamped and converted in pdf and stored in a folder. The folder should be default folder for email attachments. There may be pictures as part of the inspection report. The System should email the report to a given email address automatically upon internet connection availability and the user should be able to so do the same. Application will have built in calendar. The user should be able to schedule the tasks or inspections. The application should not allow the user to change the time or date of the previously completed inspections. Upon completion the developer will have to remotely access the internet connected laptops (3 to 5) to install and test the software and fix the bugs if any. Developer will hand over the source code of the application and will not integrate his or his company name in the application but will integrate the given name. Upon the Launch the application will display the “company name & Logo” & powered by “another Company name and logo”. Application should work with windows OS. Please suggest the programming language and the reason to use that language. I need this application in 5 days and will allow another 5 days for testing and fine tuning. Please see the form attached for the list of violations. Prepare a mockup and show me on skype or other way. This is small and easy application and does not have big budget so please be reasonable while bidding. Payment 100% upon satisfactory testing of the software. $20 perday penalty will be deducted from the developers pay after the promised delivery date.
