Magento add on fields for Shopping Cart Extension

We are currently developing an online store for eyeglasses, using Magento. While doing the checkout, we do want to give our customers to select various options before final checkout. The fields that we need to add in the product page can be seen in the attached pdf (THE YELLOW SECTION IN THE PDF IS WHAY YOU WILL BE DEVELOPING). We do want from the admin area to be able to modify all fields and images, etc... So lots of flexibility is needed from the admin area to be able to add more dropdown fields, checkboxes, images (showing lens types), etc... Depending on the type of prescription selected, we should be able to offer various types of lenses. There is also checkboxes. Also each lens type will get a different price, and shopping cart should be updated with type of lens selection and total price needs to reflect total of frame and lens prices. You must have extensive experience in developing extensions for Magento, as we will ask to see them. You must develop this for us in a short period. If you are planning to take weeks to make this for us, then do not bother bidding. We will not release you any money until the project is done and tested and working perfectly. Thanks all for bidding
